Our Nurse Support Programs provide nursing services, support, and care coordination to families with small children in Adams, Arapahoe and Douglas County. We also provide services to at risk adults in Arapahoe County only.
Our partners in County Human Services offices identify families and individuals that could benefit from Nurse Support Program services. Nurse Support Program nurses provide families and individuals with nursing assessments, care coordination and education based on needs identified by the client county caseworker and nurse.
The mission of the Nurse Support Program is to reduce the risks and mitigate the effects of traumatic events, while encouraging healthy social, physical, and emotional development. We provide a holistic assessment of individuals and families, and advocate for equity in our communities by connecting our clients to the support systems and resources they need to achieve optimal health.
Families must reside in Adams, Arapahoe or Douglas County, meet income requirements and be pregnant or parenting a baby under six months. Public health nurses provide prenatal education, resources, and parenting support up to baby’s first birthday. Complete our referral form for your patients to be seen by one of our nurses.
